DCFS Investigator Evaluation Survey Print E-mail
Make your voice heard!

The Family Defense Center has created an evaluation system to hold DCFS investigators accountable for their performance.  We want to know what you think of the job DCFS is doing--from a consumer satisfaction standpoint.  By completing this evaluation, you are making sure your voice is heard.  More than that, the FDC will use the information that you provide as data for DCFS officials to consider in our advocacy to improve and reform DCFS investigations.

Please contact Alison McIntire with any questions regarding this evaluation.

Please complete our evaluation.  You may download the form below in either Word and Adobe PDF.  You may return the form to us by email, fax or regular mail.
